There are two major differences between the two.

LikeRec:

- The file has to be defined in the program (although the file definition can be defined as TEMPLATE so that the file does not actually have to be used).
- No additional fields can be added to the DS

ExtName:

- No requirement for the file to be defined in the program
- Additional fields can be added

Personally I use ExtName for situations where there is no actual database with that layout and/or it is a base to which I will add custom fields. In all other cases I would use Likerec.
