Finally closing the loop on this one:
I opened a case with IBM and it was open for several weeks.
I sent them the image files and IBM validated that the media did fail verification. (We already knew that)

They we unable to duplicate the issue, the images they created validated.
They implied that it must be an issue with the partition I was creating the media on.
I did a scratch install of the partition with fresh downloads from ESS and created new distribution images.
Same problem.

I stumbled into what the problem was on my own.
It was the volume label on the first image...I used "I_BASE_01" for a volume label, made since to me since that is the volume label IBM uses for the LIC media.
I initialized the media and used a volume label OTHER than "I_BASE_01" and the resulting media set verified without an issue.

You could open a case.
Then again it might have an issue with *UPGRADE when that file says you're already at that release.

What OS are you upgrading from?
If you are at 7.3 there is a ptf for this message.
In the "Cause" there is supposed to be a file named for parameter 2 in
Cause . . . . . : Required file &2 for the operating system was not found
What is the name of that file?
