Well, I'd order the latest cume and groups for V5R4, just in case. I
forgot that machines still running V5R4 are considered ancient and the
media for that is assumed to be CD instead of DVD. I just ordered my last
copy of V5R4 ptf media to lock up in the vault. Came in on 3-4 dozen
CD's.
You may want to perm apply these new ptf's to get them in stone on the B
side. There may be a ptf on the existing B side that had an issue that
only hits in rare situations like the one you're experiencing. Hopefully
IBM probably came out with a fix for the fix that will fix your B side
issue.

This SRC indicates that it is a multilpar machine and you're supposed to
scan the secondary partition's SRC history list for prior B2ppxxxx failure
SRCs (where pp is the partition ID) to pinpoint the source of the problem.

Or, you could upgrade and forego worrying about V5R4 ptf's.
