I'm not sure if it will satisfy all of your requirements, but check the 
link below for CRTCSVF.
The other option is the IBM utility, CPYTOIMPF, but as of V5R3 has become 
quite buggy. Also, it will not allow you to insert the column names.

Hi, 

We have the following requirement in our project: 

1. A csv file containing text and currency fields will be generated in 
user specified folder 
2. The users want to open it in excel, so that they can slice and dice the 
amount fields. 

3. Is it possible to create a template so that when the users open the csv 
file in excel the following formating is available:

A) Report title at the top 
B) Column header row is in bold 
C) is it possible to adjust the column widths 
D) Format the currency amounts so that $amounts is displayed 

Please send me a mail if you can help me out with the above
Please inform me if there is a short tutorial / reading material which can 
be used to generate the above formatting.
