I've noticed it in manuals from IBM, Fujitsu, HP, etc. I believe it is caused by Acrobat starting with the first 'sheet' as page 1. That would be the front outside cover of a book. Page 1 according to the Table of Contents, however, is wherever the book's creator decided to start numbering the 'sheets'.
