We worked VERY closely with HR and they had a process that notified us as to when someone would be leaving, etc. (us and the building security folks, etc.) If it was immediate, they got in touch with us directly. As a "future enhancement" my plan was to have HR enter the deactivation/termination information themseleves.... I do agree that at a company with a large user base, it would be hard for help desk folks to know who they were actually talking to ! One way, although this too, is not FOOL PROOF, would be for the help desk person to call back and ask for this person (i.e. do not call a direct number, but rather the operator...). Don't have them paged but rather ask for a supervisor to get them, etc. Who knows... ?
