Call IBM Fax on Demand at 1-800-426-4329 and order document #4792.
This is from http://as400service.ibm.com/supporthome.nsf/Document/10000006

This document gives you the fax number to fax the form to after you fill it out.
1-800-288-9584.  YOu can also specify the media you wish to receive the PTFs on.
