Host Services component failure in VARPG

We have taken an existing V3R2M3 VARPG application and made modifications
using the V4R5M1 version of WDT.  The application tested fine in
development, however, after packaging and installing runtime and the new
project we received an error as soon as we tried to start up the app.  I
believe that all of the communications settings are correct.  I've copied
the error text and the output of the FVDCERRS.LOG file below. Has anyone
come across this problem before?

0940 Error occurred in host services with reason code X'00000003'.
Cause . . . . . :   An error has occurred while using a host service.
 The following reason codes may be returned: 
X'00000001' -- A communications failure has occurred. 
X'00000002' -- A DDM failure has occurred. 
X'00000003' -- Internal failure within the Host Services component.

X'00000429' -- The IP address specified in the Remote Location field
is incorrect or has not been configured for the network. 
X'000007D1' -- The host name specified in the Remote Location field is
incorrect or has not been configured for the network. 
Recovery  . . . :   Check the error log file FVDCERRS.LOG which may
contain more information on the cause of the error.
