I am seeing the same behavior.
My custom command is still there however.
I don't have any plugins installed.

I'm glad you have seen this, because I have seen the same thing.

Right after installation, there were no duplicates.

When I added my own custom command, the duplicates showed up.

It's easy to duplicate this behavior - just create your own custom command.

So I'm not crazy after all!! Well, maybe I am, but I was not wrong!!

It also looks as if 9.5.0.2 removed my custom create commands, but the duplicates are still there. All I know is, I had some, and now I don't.
