Yes, Live Outline is active.
When I click the "refresh" link, the outline does refresh, but the file
definition in the outline view does not reflect the current file.
It is not a library list issue either, as this is a new file that only
exists in one library that is in the library list of the connection I am
working in.

Does the outline refresh if you click the refresh icon (the arrows
curving toward each other)?

Is Live Outline turned on?
Windows > Preferences, search for RPG, it's in Remote Systems LPEX
Editor > IBM i Parsers > ILE RPG. In the bottom section, there's a
check box called 'Update the Outline view when text is changed in the
editor'.
