Yes, I agree with Michael.  The "Convert to Free Form" option must be
selected from the "Source" drop down menu in order for conversion to occur.
Then the changed source would have to be saved either by pressing the "Save"
button or answering "Yes" when prompted to save upon exiting.  Sounds to me
like someone did a little more than just "open the file". ;-)

Well...RPG/Free is a specific product (or was anyway), so I assume
you're just referring to free-format RPG. AFAIK, WDSc will only change
from fixed to free format when requested, either by converting the
entire source member or by highlighting a section and converting that
section.
