One other possibility that occurs to me:
- by any chance is WTEREL a logical file? A logical file with a Select/Omit line at the bottom? If so, is the logical, by definition, not including the "03" record because of some failing COMP condition?

Perhaps some other, newer program is inserting the "03" record into the file but the programmer didn't know s/he had to populate some other field in the "03" record with a specific value. That scenario would produce exactly the results you are seeing.

The problem is that the program was told to look, in its INZSR (the only
place that file for a record that wasn't there. As soon as it was no
longer being told to do that, it worked fine. And so long as the record
is there, it finds the correct one consistently.
