I've done this at a previous employer. I no longer have the code to share, unfortunately.

I've also published articles on creating the document in Word, and then going in with RPG and modifying it, maybe those would help?

But, if you do want to create from scratch, I might be able to answer questions you have, but I don't have an example to send you.
