Scott, I understand that the UPDSRVPGM/CRTSRVPGM is available to do those
things, but do I HAVE to do one/other? If I don't what happens?

The service program can be updated with UPDSRVPGM after those modules that
are part of it have been recompiled.  You can also do the CRTSRVPGM if you
want.
