RE: Address fields

Make the seven lines each text fields large enough to hold the largest thing
you have--either the address line or city,state,zip and populate them as
needed.  E.g. name in line one, adr1 in line 2, city, state, zip in line 3
for your example.  There will have to be some blank lines on the bottom for
addresses that don't have the full seven lines of data unless you want to go
to heroic measures like making every line on the page and 80 character text
field.
