With all due respect David, I think it's safe to say that most people, myself
included, don't even read the monthly list guidelines anymore.  Yes, I did
read them the first couple of times, but no more than that.

Plus, I think in some cases we're dealing with "mailing list newbies", who
don't know that they're sending HTML in the first place until someone from the
list tells them so.  And, then, it may be the case that they're not allowed
to, or are unable to, or don't know how to, turn it off.  Or perhaps setting
HTML on or off is a global all or nothing setting for some email clients.

All that said, I think it's true that some of us get a little lazy at trimming
the original content when replying.  I am conscious about it most of the time,
but if I'm replying in a hurry, I may get careless and not "clean up" like I
should.  It seems with the huge volume of posts, trying to read (glance at?)
all of them and reply to some of them, and then still have time to actually do
your job (?!?!?), it becomes rather easy to neglect some of the
responsibilities each of us have when using this list.  At a minimum, I always
try to trim the several lines that the list software adds to the bottom of
each email.  There are some posts I've seen where the thread has gone on for
quite some time and I find several iterations of that at the bottom.
Fortunately, I don't use the digest mode, where I imagine it would be most
frustrating to have to scroll through.
