I've got a Kyocera 7135 Smart Phone which is just awesome.  It is
both a Palm and Cell Phone.  In addition, it has an SD Memory
card slot and is an MP3 player.

It is a little larger than today's phones, but I only carry the
one device.  The belt clip is very good.

The only downside?  Cost.  About $500.  You can probably get it
cheaper nowadays, but it is not inexpensive.
