The key there is "Usually". Sure a bank vault probably has to have some.
But OSHA's never dinged us. Maybe because the door can always be opened
from the inside. Sort of like any basic office. I could see a need for
ventilation if your door could only be opened twice a day with no
overrides, rivaled NORAD's door in thickness, etc.
