Do note that depending on WHY it has issues, continued attempts to read it may result in damage to the drive itself. Recently had a customer literally grind a tape to bits in their LTO4 drive thus requiring its replacement.

The only 'fix' I have ever seen is to replace the metal bit on the end of the tape. Beyond that there is no 'fixing' of the tape. In your case if it's reporting 'Media Errors' then it's loading so its not the metal bit.

Time for a new tape I'm thinking.
