Look up "fillable form pdf". Fillable Form was an addition to the standard pdf (I say standard, pdf is a defacto proprietary standard) about 5 years ago, maybe more. Fillable Form PDF files can be "edited" in the standard reader, but only those fields that are designated as fillable can be changed. The user can save the file with the entries. Once saved, he could mail it back.
Most of the pdf generators, iText, for example, do not support fillable forms. On the other hand, iText is open source and some resourceful folks may have added this since I last downloaded iText.
