I would do a manual IPL back into DST, and you will have an option to
install the operating system. I would do this, and make sure that it
completes.

I am not familiar enough with the OS loading procedure to know if the
media error is with the CD or now it can't be read from disk.

Perhaps a call to IBM support? I saw one APAR but it had to do with a
network install, which doesn't apply.
