Unique Constraint over all 4 fields would work and is the 'easiest'
solution.
I don't think Check Constraint would work... well, perhaps with some sort of
UDF it might... something to test.

Subject: SQL Table / DDL question on using Unique keyword

Is it possible to set it up so that I have to have a unique combination
of
fields. Example I have four fields, Employee first name, Employee second
name, city, and state. I do not want a BOB Smith in the same city and
state but I do not care if I have twenty Bob Smith's in the file.
