Do you have the "problem analysis, repair, and parts" book? Look under "recovery procedures." I note that the last couple of drive swaps I did went quite a bit better than previous ones, and unlike some previous drive swaps, it didn't leave the failed drive in the listings as "missing." I'm pretty sure I did something differently from the way I'd done it before, but at the moment, I can't recall exactly what, except for having the appropriate manual pages handy.

Also, note that according to the aforementioned manual, "it can take up to 5 minutes for the IOP to report the device to the system."
