I believe that would indicate a non-recoverable line drop, i.e. modems lost 
connection.

Does anyone know what could cause ICF file to return a 8191 major/minor return 
code?  
 
Below is a description about 8191 from IBM manual:
 
Description: The operation was not successful because a permanent line error 
occurred, and the system operator took a recovery option in response to the 
line error message. (You can find out what type of line error occurred by 
asking the system operator.) The device has been marked unusable.

Action: Release the device in error or close the file. Vary the device off and 
on again to clear the error. Open the file again or acquire the device to 
continue.
