Support!  It all falls onto a small number of people, usually 1 - 3, in a
small shop and they have to know everything.  Larger shops usually have
specialized people for individual components, servers, and applications.

User mind set.  Small shop they know everything and in the large shop they
think they know everything ;-)

Large shop has good support contracts for everything but the small shop may
have limited support contracts.
