I defer to you being an expert in this area, but I have not had any trouble
using SMTP to my phone for monitoring.  Sure, I am not a Dr. (I did stay at
a Holiday Inn Express last night), but I have found this to be reliable.  As
a check, I have one of my AS/400 send my phone a "check in" message twice a
day (8am and 8pm) and it is so reliable and immediate, I can set my watch by
it.

         Yes, in the Americas we can number@carrier.com SMS to most 
cellular providers.  BUT, they is no audit trail, there is no urgency, and 
there is no way of knowing which of any number of relay points is 
overloaded with SPAM.  Using SMTP to deliver messages in any type of 
monitoring application should be the LAST resort not the first.  Protocols 
that might be available are are all more reliable include SMPP, WCTP, SNPP, 
TAP and for large users like hospitals, sometimes TNPP direct links.
