This is a known problem within IBM.  Had the same issue doing a V4R4 to V5R1
upgrade last month on a Model 720 2063.  Wound up having to do the install
of Licened Program Products manually........I not only raised hell with the
support center, but also pushed this through another channel as a post
install review.  Have heard nothing back as of yet......................

You are right on the mark about much end user responsibility that is not
documented in the install guide or memo to users.  If anyone else has
experienced this, please, please log your complaint with IBM!!!!!!  Maybe if
enough do that, this will get fixed.................
