Years ago (pre 400, in fact pre 38) when I worked at Caterpillar Tractor, 
our department was running two 360's in a glass room with an automatic door 
controlled by a large button on the wall.  When you wanted to go in or out, 
you just hit the button and the doors opened.

Well, one night a new guy started work, and when he went to go out with a 
cart full of tape reels (remember those!), he hit the button on the wall, 
but it was on the other wall instead - which was the emergency power cut off 
for the data center.  It took about 8 hours to get both machines back up 
again.

The next day the department manager was there and asked the guy what 
happened.  He said "I was going out with some tapes, and I hit this button 
over here...", and believe it or not, he hit it again while he was telling 
him!!!!

No he didn't get fired, but a few years later they made him a manager.  You 
can't have dangerous people like that hanging around systems!
