I have used both CreatePrint and Optra Forms.

Both require Postscript printers but Optra Forms required a Lexmark Optra
printer.

Optra forms is completely PC based. You create your forms and download all
graphics and form data to the printer.

This reduces network traffic when you print your forms.

I have used Create!print WWW.Createprint.com, Postcript Only does a good
job.
and FormMagic WWW.PROHELP.COM, PCL Only a little more difficult to use, but
it costs around $700.00.

Depends what you are trying to achieve.
