I had the same problem, you have to force the SMTP server to do a cold
start.
Create a data area called QTMSCLEAN with this command.

CRTDTAARA DTAARA(QUSRSYS/QTMSCLEAN) TYPE(*CHAR) LEN(1) VALUE('c') AUT(*ALL)

Then restart SMTP, when SMTP restarts all unprocessed mail will be lost.

If you want to do a warm start change VALUE('c') to VALUE('W'), this will
SMTP to
clear only the first mail control block.

This may not work for you but your symptoms sound the same as mine.
