Boy THAT is for sure !! Years back at a place I was working, we had a line of
twinax that had been working FINE. 3 devices. We go to add a 4th and EVERYTHING
starts going nuts. The 4th device was fine the 2nd one on the line failed, etc. 
We
THOUGHT we had checked EVERYTHING and gave up and called IBM. Our CE came out 
and
after about an hour he found the problem - reverse polarity (sp ?). On the
insulator there is a mark and you should be careful to consistenly aline either
the "copper" colored or "zinc/silver" colored wire/pin with this. Our 
maintenance
folks HAD been doing this - it turned out that our supplier had managed to 
produce
a WHOLE batch of these "mismarked" and unfortunately no on notice that the 
"notch"
in the insulator had" changed sides in relation to this mark !!
