My apoligies for not making this clear.  In the command, only the parameters 
with ? are
the ones shown on the screen for user input.  For your example then;

To forward:
chgdire usrid(test01 patvl) usrdfnfld((forwarding *none test01@usa.net))
msfsrvlvl(fwdsrvlvl) prefadr(forwarding *n atmime)

To cancel forwarding:
chgdire usrid(test01 patvl)  usrdfnfld((forwarding  *none  '  ')) 
msfsrvlvl(*sysms)
prefadr(*smtp  *n  smtp)
