This is kinda old (the days when I had time to read such magazines have
long ago passed away) and there are probably new ones out there, but the
News 3X/400 April 1992 issue had an article that got me started.  It is
really very easy (except of course the QMFORM parts).

All I've ever seen is the delightful "OS/400 DB2 for OS/400 Query
Management
Programming" referred to above as "TFM."  The section on QMFORMs is,
even
for IBM, unusually obscure.  What you might try is creating a QMFORM
from a
Query400 query using RTVQMFORM and using it as a template.  If you're
like
me, you won't be pleased.  There's all sorts of arcane arbitrary stuff
to
learn if you have the time and the inclination.  

If you haven't done it yet, you can convert Query400 queries to QMQRY
source
too using RTVQMQRY.  I've used this a lot since we don't have the full
SQL
package.  It's the easiest way to pass parameters to a query in my
environment.
