Another Vendor that supports form overlays are Formula One
(1-800-603-6768).  

Here are some instructions to create a AFP overlay on a PC and convert on
the AS/400

Here are the steps to follow:

1. Install the PC Support AFPDS print driver (this is designed for Windows

2. Use the Control Panel to connect this driver to FILE (so it will "print"
to a file instead of an LPT port)

3. Create the image (or scan it).

4. Print the image to the AFPDS driver; you will be prompted for a file
name.

5. Transfer the file to the AS/400, putting it into an empty physical file
w/ a
record length of 1024 bytes.

6. Use the Create Page Segment (CRTPAGSEG) or Create Overlay (CRTOVL)
command
to convert the AFPDS file to the appropriate Page Segment (*PAGSEG) or
Overlay (*OVL) object. 

7. The image is now on the AS/400, ready for use by RPG, DDS or the OVRPRTF
command.
