Check to see if your PreparedStatement or your Connection was previously
used and then closed but you're now attempting to reuse without getting
a new instance from your Connection pool or getting a new.

If that doesn't help, you may be able to turn on tracing for
com.ibm.ejs.cm.pool.* and see more about what is happening.
