Well, this is the 'workaround' I am settling for:
- File, Database, Properties, Design tab, turn off 'Inherit design from
template'.

When I put on a new version of Domino then I'll do the following
- File, Database, Properties, Design tab, turn on 'Inherit design from
template'.
- File, Database, Refresh design
- Replicate the nab to all servers.
- Run the fax install to get the tab information back.
- File, Database, Properties, Design tab, turn off 'Inherit design from
template'.
- Replicate the nab to all servers.
- Revisit the debate as to why to run 'combined' versus 'separate'.
- Get anesthetized
