You would do two things. You'd try to shoot the missile down, and
you'd launch a retaliatory strike. You'd have to. That's how the
theory of MAD works. The enemy doesn't attack you because they are
100% certain you will hit back. If the enemy thinks there's the
smallest possibility you are bluffing the whole thing falls down.

The downside is that you must have decided in advance that if they do
attack you, you will hit back. That's how you make sure they don't
attack you. It's a mathematical decision (game theory), not a moral
one.
