First question - are you sure you have all current PTFs applied for both
database and COBOL (I'm assuming ILE COBOL from the errors you are getting).

If you are up to date, then have you re-compiled any or all of the programs?
Have you read the Notice to Users to see if COBOL pre-reqs any changes for
V5R1?

FS 41 is typically caused when a COBOL thinks the file is closed and the
system doesn't - i.e. the ODP still exists in the AG.  Typically happens in
an ILE/OPM mix which wasn't thought through properly.  FS 92 can arise under
similar circumstances.  For 90 and 91 they are pretty meaningless without
knowing the underlying CPF messages.
