I have seen this issue even in V8.1. It occasionally happens in our 
environment irrespective of item being lot controlled or not in pick 
confirmation program when you are trying to fiddle with allocation 
(ORD720D*) programs both in managed and regular warehouse. Check your 
allocation required flag in item master and also in system parameters to 
make sure if any exceptions defined. If every thing is setup correctly ask 
your end users if they were doing some allocation revision within pick 
confirmation program.

I have seen orders going to invoice correctly without actually creating 
ITH records. All inventory files updated properly too. Only ITH record is 
missing.

Thanks for the suggestions. Unfortunately, the item is not stock 
controlled,
there was enough stock for about 10 days prior to the transaction 
occurring
and the stocking/selling UOM's are both EA with conversion factor of 1.00
I think this is going to be another "just amend the quantities and see if 
it
happens again" issue.
