A SSA GT representatives or a business partner can provide you with this
form. It is then processed by the IBM ERP center to determine the proper
sized box. The form was developed in cooperation between SSA GT and IBM and
is very detailed. SSA GT can also assist you in filling out the form if you
are unsure of some of the questions.
